The reason that violations of the LOTO Standard are among the Top 5 most frequently cited Standards every year is that employers must self-report work-related amputations and admissions to the hospital within 24 hours. As a consequence, it is likely that such cases often involve an employee's failure to lock out or the employee’s decision to reach past guarding into a machine while the machine is still running.
These cases often result in OSHA conducting an on-site inspection and ultimately result in citations alleging violations of the LOTO standard. It is, therefore, crucial to understand how OSHA interprets an employer’s compliance obligations under the LOTO Standard.
